I need to Add a HyperLink to my Form
You can use HTML in the header and separation element and in field captions, to add hyperlink and additional custom styles to  your form. Just insert the HTML into the header field or field caption. Use single quotes as in this HREF example:  <a href='http://www.cnn.com' target='new'>Click here for CNN</a>  We also recommend that when using links that you use the target='new' attribute to open the link in a new window.
