I understand and acknowledge that the sport of Disc Golf may involve physical risk. I accept the responsibility for inspecting each area, course or field on which I play Disc Golf in connection with the Ottawa Disc Golf Club and satisfying myself as to its safety.

In consideration of the Ottawa Disc Golf Club and Ontario Disc Sports Association ("ODSA") accepting me as a participant during the 2008/2009 season, I, for myself, my heirs, executors, administrators, successors and assigns, hereby release, waive and forever discharge the Ottawa Disc Golf Club and ODSA, its sanctioning body and sponsors and all their respective agents, servants, contractors, representatives, directors, elected and appointed officials, successors and assigns, of and from all claims, demands, damages, costs, expenses, actions and causes of action, whether in law or in equity, in respect of death, injury, loss or damage to my person or property howsoever caused, arising to or to arise by reason of my participation in the 2008/2009 season of the Ottawa Disc Golf Club, whether as a spectator, participant, competitor or otherwise, whether prior to, during, or subsequent to the 2008/2009 season and notwithstanding that same may have been contributed to or occasioned by the negligence of any of the aforesaid.

I further hereby undertake to hold and save harmless and agree to indemnify all of the aforesaid from and against any and all liability incurred by any or all of them arising as a result of, or in any way connected with, my participation in the 2008/2009 season of the Ottawa Disc Golf Club.

By accepting this waiver, I acknowledge having read, understood and agreed to the above release, waiver and indemnity. I warrant that I am physically fit to participate in the 2008/2009 season of the Ottawa Disc Golf Club.

ONTARIO DISC SPORTS ASSOCIATION & SOUTH RIVER DISC GOLF CLUB

In consideration of the Etobicoke Disc Golf Enthusiasts and Ontario Disc Sports Association ("ODSA") accepting me as a participant during the 2008/2009 season, I, for myself, my heirs, executors, administrators, successors and assigns, hereby release, waive and forever discharge the Etobicoke Disc Golf Enthusiasts and ODSA, its sanctioning body and sponsors and all their respective agents, servants, contractors, representatives, directors, elected and appointed officials, successors and assigns, of and from all claims, demands, damages, costs, expenses, actions and causes of action, whether in law or in equity, in respect of death, injury, loss or damage to my person or property howsoever caused, arising to or to arise by reason of my participation in the 2008/2009 season of the Etobicoke Disc Golf Enthusiasts, whether as a spectator, participant, competitor or otherwise, whether prior to, during, or subsequent to the 2008/2009 season and notwithstanding that same may have been contributed to or occasioned by the negligence of any of the aforesaid.

In consideration of the Bronte Creek Disc Golf and Ontario Disc Sports Association ("ODSA") accepting me as a participant during the 2008/2009 season, I, for myself, my heirs, executors, administrators, successors and assigns, hereby release, waive and forever discharge the Bronte Creek Disc Golf and ODSA, its sanctioning body and sponsors and all their respective agents, servants, contractors, representatives, directors, elected and appointed officials, successors and assigns, of and from all claims, demands, damages, costs, expenses, actions and causes of action, whether in law or in equity, in respect of death, injury, loss or damage to my person or property howsoever caused, arising to or to arise by reason of my participation in the 2008/2009 season of the Bronte Creek Disc Golf, whether as a spectator, participant, competitor or otherwise, whether prior to, during, or subsequent to the 2008/2009 season and notwithstanding that same may have been contributed to or occasioned by the negligence of any of the aforesaid.

In consideration of the Brantford Disc Golf Club and Ontario Disc Sports Association ("ODSA") accepting me as a participant during the 2008/2009 season, I, for myself, my heirs, executors, administrators, successors and assigns, hereby release, waive and forever discharge the Brantford Disc Golf Club and ODSA, its sanctioning body and sponsors and all their respective agents, servants, contractors, representatives, directors, elected and appointed officials, successors and assigns, of and from all claims, demands, damages, costs, expenses, actions and causes of action, whether in law or in equity, in respect of death, injury, loss or damage to my person or property howsoever caused, arising to or to arise by reason of my participation in the 2008/2009 season of the Brantford Disc Golf Club , whether as a spectator, participant, competitor or otherwise, whether prior to, during, or subsequent to the 2008/2009 season and notwithstanding that same may have been contributed to or occasioned by the negligence of any of the aforesaid.

In consideration of the Ontario Disc Sports Association ("ODSA") accepting me as a participant during the 2008/2009 season, I, for myself, my heirs, executors, administrators, successors and assigns, hereby release, waive and forever discharge the ODSA, its sanctioning body and sponsors and all their respective agents, servants, contractors, representatives, directors, elected and appointed officials, successors and assigns, of and from all claims, demands, damages, costs, expenses, actions and causes of action, whether in law or in equity, in respect of death, injury, loss or damage to my person or property howsoever caused, arising to or to arise by reason of my participation in the 2008/2009 season of the disc golf, whether as a spectator, participant, competitor or otherwise, whether prior to, during, or subsequent to the 2008/2009 season and notwithstanding that same may have been contributed to or occasioned by the negligence of any of the aforesaid.

I accept the responsibility for inspecting each area, course or field on which I play Ultimate in connection with the Barrie Ultimate League and satisfying myself as to its safety.

In consideration of the Barrie Ultimate League and Ontario Disc Sports Association ("ODSA") accepting me as a participant during the 2008/2009 season, I, for myself, my heirs, executors, administrators, successors and assigns, hereby release, waive and forever discharge the Barrie Ultimate League and ODSA, its sanctioning body and sponsors and all their respective agents, servants, contractors, representatives, directors, elected and appointed officials, successors and assigns, of and from all claims, demands, damages, costs, expenses, actions and causes of action, whether in law or in equity, in respect of death, injury, loss or damage to my person or property howsoever caused, arising to or to arise by reason of my participation in the 2008/2009 season of the Barrie Ultimate League, whether as a spectator, participant, competitor or otherwise, whether prior to, during, or subsequent to the 2008/2009 season and notwithstanding that same may have been contributed to or occasioned by the negligence of any of the aforesaid.

In consideration of DIRT and Ontario Disc Sports Association ("ODSA") accepting me as a participant during the 2008/2009 season, I, for myself, my heirs, executors, administrators, successors and assigns, hereby release, waive and forever discharge DIRT and ODSA, its sanctioning body and sponsors and all their respective agents, servants, contractors, representatives, directors, elected and appointed officials, successors and assigns, of and from all claims, demands, damages, costs, expenses, actions and causes of action, whether in law or in equity, in respect of death, injury, loss or damage to my person or property howsoever caused, arising to or to arise by reason of my participation in the 2008/2009 season of DIRT, whether as a spectator, participant, competitor or otherwise, whether prior to, during, or subsequent to the 2008/2009 season and notwithstanding that same may have been contributed to or occasioned by the negligence of any of the aforesaid.

I further hereby undertake to hold and save harmless and agree to indemnify all of the aforesaid from and against any and all liability incurred by any or all of them arising as a result of, or in any way connected with, my participation in the 2008/2009 season of DIRT.

By accepting this waiver, I acknowledge having read, understood and agreed to the above release, waiver and indemnity. I warrant that I am physically fit to participate in the 2008/2009 season of DIRT.

In consideration of the Durham Ultimate Club and Ontario Disc Sports Association ("ODSA") accepting me as a participant during the 2008/2009 season, I, for myself, my heirs, executors, administrators, successors and assigns, hereby release, waive and forever discharge the Durham Ultimate Club and ODSA, its sanctioning body and sponsors and all their respective agents, servants, contractors, representatives, directors, elected and appointed officials, successors and assigns, of and from all claims, demands, damages, costs, expenses, actions and causes of action, whether in law or in equity, in respect of death, injury, loss or damage to my person or property howsoever caused, arising to or to arise by reason of my participation in the 2008/2009 season of the Durham Ultimate Club, whether as a spectator, participant, competitor or otherwise, whether prior to, during, or subsequent to the 2008/2009 season and notwithstanding that same may have been contributed to or occasioned by the negligence of any of the aforesaid.
